Office Procedures & Policies
Thank you for choosing City Centre Obstetrics and Gynecology. Our goal is to provide the best possible medical care and to develop a relationship with you. To successfully maintain this relationship, we want you to have a clear understanding of our office procedures and policies.
First Visit
Bring your MSP card and ID. You will need to pay for the fees rendered if you do not have MSP coverage.
You will be asked to provide your home address, phone number, email address, and name of your family doctor.
Office & Parking
We are located in tower one on the 12th floor.
Parking: There are four levels of pay parking available, with a pay kiosk by each elevator. It's a big place, so make sure to give yourself extra time to find and pay for parking. We suggest paying for at least one hour of parking.
Skytrain: It’s a short walk from the King George Skytrain station and located directly across from Surrey Memorial Hospital.
Appointments
A referral from your family doctor is required to get an appointment. We do not accept walk-ins.
Cancellation Policy
If you need to cancel your in-office or telehealth appointment, we respectfully request at least 48 hours of notice. Any cancellation made in less than 48 hours will result in a cancellation fee.
Form Completions
If you have letters or forms for our doctors to complete, please know that there is an administration fee for each form as MSP does not cover them.
Respectful Environment
We maintain an environment of mutual respect and do not tolerate verbal or physical abuse. Angry outbursts against our staff may result in your discharge from the practice.
